Plastic and Hand Surgery of North Texas has updated our insurance plans and policy as listed below. We are no longer accepting the following primary OR secondary insurance plans: HMO or any PPO plans requiring a referral, Tricare Prime (requires referral), Champ VA, Humana, Molina, Ambetter, Bright Health Plans, Medicaid, Marketplace plans, Wellmed, Healthsprings, Southwest Health System, Medishare (***This list may not be complete. See list of insurances that we DO accept below).
NOTE: We REQUIRE insurance information and verification that your insurance is active AND in-network PRIOR to making an appointment. A copy of your insurance card(s) and your government I.D. are required at check-in. Your name and birthdate must match your insurance card in order for us to accept your insurance.
***IN ADDITION: Insurances may tell you our provider is IN-NETWORK. However, we opt to not participate in certain plans such as HMO, PPO requiring referrals, certain Medicare advantage plans (i.e. HUMANA, WELLMED, OSCAR). Please call our office to verify coverage information prior to scheduling your visit/procedure. Some restrictions and exclusions may apply, as we may not be immediately aware of changes implemented by insurance companies.
It is ultimately the patient's responsibility to check if your insurance is IN-NETWORK with our office, the surgery center, and with the anesthesia group.
Copays, coinsurances, and deductibles, if applicable, are required by your insurance to be collected. Please contact your insurance carrier to understand your coverage, copay , deductibles, and out-of-pocket and referral requirements.
We accept most major credit cards and except exact cash payments only. We do not accept checks and care credit as forms of payments.
We do NOT accept Workers Comp cases, motor vehicle accident cases, or liability accident cases.
Existing patient accounts, dependents, guarantors, and insurer's account (s) must ALL be in good standing with zero balance prior to making appointments.
