Insurance Information

Plastic and Hand Surgery of North Texas has updated our insurance plans and policy as of 1/1/2022 as listed below. We are no longer accepting the following primary OR secondary insurance plans: HMO or any plans requiring a referral, Tricare Prime, Champ VA, Humana, Molina, Ambetter, Bright Health Plans, Medicaid, Marketplace plans, Wellmed, Healthsprings, Southwest Health System (***This list may not be complete.  See list of insurances that we DO accept below).

***Please call our office AND your health plan to verify coverage information prior to scheduling your visit/procedure.  Some restrictions and exclusions may apply, as we may not be immediately aware of changes implemented by insurance companies.  

NOTE: We REQUIRE insurance information and verification that your insurance is active AND in-network PRIOR to making an appointment.  A copy of your insurance card(s) and an I.D. are required at check-in.  

We will do our best to check if your insurance is IN-NETWORK with our office, however, it is ultimately the patient's responsibility to check if your insurance is IN-NETWORK with our office, the surgery center, and with the anesthesia group.  

Copays, coinsurances, and deductibles, if applicable, are required by your insurance to be collected. Please contact your insurance carrier to understand your coverage, copay , deductibles, and out-of-pocket and referral requirements.  

We accept all major credit cards, bank checks, and cash.  

We do NOT accept Workers Comp cases, motor vehicle accident cases, or liability accident cases.

Existing patient accounts must be in good standing prior to making appointments.
